Trauma-Sensitive Mindfulness: Providing A Choice of Anchors

David Treleaven recently published a book on Trauma-Sensitive Mindfulness. The book enables mindfulness trainers to recognise a trauma-affected individual, provide appropriate modifications to their mindfulness processes and avoid aggravating the individual’s trauma experience.

David argues that two factors are foundational to trauma-sensitive mindfulness, (1) choice and (2) anchors.  He observes that people who are trauma-affected have experienced an unwanted negative event that endangered them, a total loss of control over the situation and a lack of agency (capacity to influence the outcomes).  Providing choice, especially in relation to anchors, is critical for the welfare of the trauma-affected individual – it avoids reactivating the sense of helplessness associated with the traumatic event and reduces the likelihood of triggering a painful “body memory”.

Providing a choice of anchors – internal sensations

An anchor enables an individual to become grounded in the present moment despite being buffeted by distractions, negative self-stories or endless thoughts.  The choice of an anchor is a very personal aspect of mindfulness – it relates to an individual’s preferences, physical capacity and emotional state.  An anchor enables a person to experience ease and emotional stability.

Jessica Morey, an experienced teacher of trauma-sensitive meditation, begins a meditation training session by offering participants a choice of three internally-focused anchors – a bodily sensation, attention to sound within their immediate environment (e.g. the “room tone”) or a breath sensation (air moving through the nostrils, abdomen rising and falling or movement of the chest).

Participants are given the opportunity to try out these different anchors over a five-minute period and to make a choice of an anchor for practice over a further period.  Providing this choice of anchors avoids locking individuals into a mindfulness process that can act as a trigger for reexperiencing trauma, e.g. sustained focus on breathing.

Alternative anchors – external sensing

David notes that the five senses offer further choices of anchors – in addition to the internally focused anchors suggested by Jessica.  The senses enable a participant in meditation training to focus on some aspect of their external environment:

  • Hearing – tuning in to the external sounds such as birds singing, the wind blowing or traffic flowing past.  The downside of this approach is that it may trigger our innate tendency to interpret sounds and this may lead to focusing on a particular sound – trying to identify it and its potential source. So, this may serve as a distraction pulling us away from experiencing (the “being” mode) to explaining (the “thinking” mode).  The aim here is to pay attention to the experience of hearing, not to focus on a single sound. Sam Himelstein has found that listening to music can be a very effective anchor for a person who is in a highly traumatised state – choosing music that aligns with the individual’s musical preferences can serve as a powerful anchor.
  • Touch – a trauma-affected person could have an object, e.g. a crystal or a stone, that provides comfort and reassurance and enables them to become grounded in the present moment through the sensation of touch.
  • Seeing – taking in the natural surroundings, e.g. by observing closely the foliage of a tree – its colours, shape and texture or observing the patterns in the clouds.

Other options include sensations of smell or taste.  However, in my view, these tend to be less neutral in character and can re-traumatise a trauma-affected person.

David Treleaven offers a wide range of resources to help meditation trainers build their awareness, skills and options in the area of trauma-sensitive mindfulness (TSM).  These include an online training course, interview podcasts, a TSM Starter Kit (incorporating an introductory video and a comprehensive “TSM Solutions Checklist”) and a live meetup of the TSM Community (registered members of a community of TSM-aware practitioners).

Reflection

As we grow in mindfulness through meditation, research and reflection, we can become more flexible about how we offer mindfulness training.  A trauma-sensitive approach to mindfulness requires an awareness of the manifestations of trauma and post-traumatic stress disorder (PTSD), identification of different sources of anchors and the willingness and capacity to offer participants the choice of an anchor and an approach to mindfulness.  This means that we need to move beyond our own fixation with “meditation logistics” and be flexible enough to offer trauma-informed mindfulness practices.

How Can We Manage When Our Daughter or Son is in Pain?

, Creator of the Open Heart Project, addresses this question in response to an inquiry from one of her many followers. The danger when someone very close to us is suffering, is that we are tempted to take on their pain, to be so empathetic that we treat their suffering as if it is our own.

This identification with the sufferer was the very problem raised by Susan’s follower in relation to her daughter’s pain:

How do we prevent ourselves from hurting on behalf of the other person?…. Her pain feels like my pain, and makes me so upset and sad. 

Susan’s response is given by way of a brief input and a guided meditation. She asserts that you cannot prevent yourself from feeling the pain of someone close to you – to do so would stop you from feeling anything. You would effectively turn off your feelings to protect yourself but in the process destroy what makes us essentially human – the capacity to feel and be compassionate.

The damaging effects of closing your heart to pain

Susan uses the analogy of a gate which has two positions – open and closed. So our heart, or our feeling with and for another, tends to be in one or other of these positions – either open hearted or closed. Susan deliberately called her life’s work the Open Heart Project because it is essentially designed to help people to open up to the full range of their experience – beauty and darkness, happiness and pain, freedom and restraint.

Susan paints a graphic picture of the difference between an open heart and one that is closed by describing the difference as that “between awake and asleep, alive and numb, present and deluded”. She suggests, however, that you cannot just be totally identified with the other person’s pain – you have to be able to achieve a separation from the other’s suffering – not own the suffering of another. Richard Davidson describes this capacity as “social cognition” which his research into the science of compassion demonstrates is essential for the “balance and welfare” of the person observing the suffering.

Susan cautions that we need to be conscious of the “toll” that feeling for another’s suffering has on ourselves. In her view, shutting off our own pain to protect our self is really self-damaging because it numbs us. The way forward is to feel the pain but actively engage in genuine self-care, whatever form that can take for you personally (this could involve exercise, yoga, Tai Chi, meditation, prayer, time with family and/or friends, accessing social/professional support or a combination of these).

Managing through compassion meditation

One of the benefits of being able to manage the pain you experience when your daughter or son is suffering is that it lays the foundation, or “pathway” as Susan describes it, to genuine compassion for others. This capacity for genuine compassion can be further developed through different forms of compassion meditation. Daniel Goleman and his neuroscience colleagues have demonstrated through research that compassion meditation develops in people an “altered trait” that is evidenced through increased kindness and generosity.

Compassion meditation, often described as loving kindness meditation, frequently begins with extending kind thoughts to someone close to you, progressing to an acquaintance, to someone you have heard about or a group of people experiencing some form of suffering and finally taking in someone you find difficult. This expanding expression of compassion can be underpinned by self-compassion meditation.

As we grow in mindfulness, we can become increasingly aware of, and sensitive to, the pain and suffering of those close to us. If we shut off these empathetic feelings, we can numb ourselves to the full range of human experience and prevent ourselves from expressing our feelings. Active self-care is essential to manage the personal toll of being empathetic and maintaining an open heart. Compassion meditation can build our capacity to sustain compassionate action not only for those closest to us but to everyone, whether we like them or not.

Recognition of the Signs of Mental Illness and How to Intervene

In the previous post, I discussed being mindful of mental health in the workplace.  This involves not only awareness and being present to staff and colleagues, but also being able to recognise the early warning signs of mental illness and having the courage and competence to intervene.

The early warning signs of mental illness in the workplace

Recognition of the early warning signs of mental illness enables early intervention to prevent deterioration in a person’s mental health.  Without such an intervention, issues can build up for the individual, making it more difficult for them to manage their stress and/or stressors.

The Mentally Healthy Workplaces Toolkit introduced in the earlier post provides a list of possible early warning signs of mental illness and lists them under five categories:

  1. Physical – such as constant tiredness, continuous ill health, major changes in appearance and/or weight, complaints about ongoing health concerns
  2. Emotional – such as irritability, loss of a sense of humour or of confidence, increased cynicism, nervousness, overly sensitive to perceived or real criticism
  3. Cognitive – overall performance decline through lots of mistakes, lack of concentration and/or inability to make decisions (constant procrastinating)
  4. Behavioural – behaving out of character by becoming more introverted or extroverted, withdrawing from group activities, lateness to work, not taking scheduled breaks (such as lunches) but taking unofficial time off
  5. In the business – inability to meet deadlines, declining motivation, frequent absences, working long hours unproductively.

There may be multiple causes for one or more of these early signs to occur.  So, it becomes important to check in with the person involved as to how they are going and whether you can be of assistance.

Checking in – having the conversation

Often managers and colleagues are reluctant to say anything to the person showing early sings of mental illness and the person involved is often unwilling to raise the issue for fear of being seen as “not coping” or “being weak”.  Part of the problem is that they really need support and care and genuine concern for their welfare.  They can be experiencing a strong sense of isolation, lack of support and associated depression.  Extending a helping hand can often work wonders.   But how do you start the conversation?

People in the workplace are very ready to ask someone about a physical injury such as a broken wrist but when it comes to a mental illness they are often fearful or uncertain – yet the person with the early signs really needs someone to show care and concern.  So, we can have a situation where the two parties – the manager/colleague and the person experiencing mental illness – are compounding the problem by not engaging in the conversation- a form of mutual withdrawal.

The recognised format for the initial conversation where someone is displaying the early signs of mental illness is called AYOK – “Are you okay?” The Mentally Healthy Workplaces Toolkit offers four steps for starting the conversation:

  1. Ask R U OK?
  2. Listen without judgment
  3. Encourage action
  4. Check in

It is useful to preface this conversation with the observation, “I have noticed that…and I am concerned for your welfare.”  In other words, communicate what you have observed (shows you are interested in the person) and express care and concern.

The person involved may be unwilling to talk initially but it is important to undertake the occasional check-in.  An experienced practitioner at the 19th International Mental Health Conference mentioned that on one occasion he had the initial AYOK conversation and the person involved said they were okay…and yet, some months later they came up to the practitioner and said, “I’m not okay, my daughter committed suicide three months ago – can you help me?”  Having had the initial conversation opened the way for the subsequent voluntary disclosure.  To avoid the conversation compounds the sense of isolation of the individual involved – they feel that they can’t help themselves and that no one else is willing to help them.

It is important to prepare for the conversation beforehand – know what you are going to say, allow time for the interaction and choose an appropriate time and place.  You need to ensure that you are prepared to listen and be mindful during the conversation.

You can provide support by suggesting they use the Employee Assistance Program, visit their doctor (who can initiate a formal Mental Health Care Plan) or discuss options for making reasonable adjustments to their work situation.  The important thing is that you take compassionate action, not letting the situation deteriorate.

It is vitally important to maintain confidentiality about any information disclosed to protect the privacy of the person involved.  You will need the explicit consent of the individual to disclose the information to co-workers, for example.  The information conveyed to you can only be used for the purpose intended by the disclosure – e.g. to enable a reasonable adjustment to their workload or pattern of work.

The exception would be where the person discloses that they are experiencing suicidal thoughts or feelings.  In this case, you will need to seek professional support.  Beyond Blue has some very sound and detailed guidelines for the conversation in these situations, including what language to use.  ConNetica, in their blog post Chats for life APP, also provides an App (with practical conversation tips) which has been designed by young people for young people experiencing mental health problems, and possibly suicidal thoughts and feelings.

As we grow in mindfulness through meditation and reflection, we can become more aware of the early signs of mental illness, have the courage and confidence to have the AYOK conversation and a willingness to take compassionate action.
